Central West Community Care Access Centre (CCAC)


May September 2014
Worked as a Team Assistant for three different hospitals, Brampton Civic, Etobicoke
General and Headwaters. In this role I processed referrals, organized contracts with
external agencies, worked with care coordinators, answered the phones and triaged the
calls to the appropriate care providers.
Processed orders, utilized the meditech system for data retrieval and used the patient
database to enter patient data into a confidential database.

Working as a Customer Service Representative, I report to the Supervisor of Service

Delivery. Act as the main point of contact for the general public, in addition to Corporate
Contacts in our Corporate Building.
Responsibilities include accepting payments for a variety of public services, such as,
water billing, child care and TransHelp. I refer the public to a variety of different
services, provide direction and am responsible to assist with problem resolution.

Worked with the hospital Ethicist, Dr. Paula Chidwick co-leading a wide variety of
different projects such as brochures for patients and families, posters for events
and national conferences, web site design, and analyzed ethicist consultation
data for organizations performance metrics.
Worked with one of the medical directors from the Intensive Care Unit
and the Ethicist to lead a research initiative which entailed development
and implementation of a checklist for meeting ethical and legal obligations.
Following from this research, an abstract was developed, submitted and
accepted at the Critical Care Forum with myself as the primary author.
Worked with the hospital Diversity Coordinator and Diversity team
to plan and host events, create displays, enhance staff awareness
related to the importance of diversity within the organization.
Was invited to attend multiple committee meetings with the Diversity Coordinator.

Worked with physicians, nurse practitioners, administrative assistants and patients to
improve their experience with care delivered.
Was responsible for a variety of tasks including: patient scheduling, OHIP billing as well
as organizing and updating patient health files.
Worked as an administrative assistant to the head physician of the health team to assist in
a variety of independent and group projects.
Was responsible for introducing patients and signing them up for miDASH an online
health portal that provides patients with easy access to their health information.
Created a health network using WebBrain as a tool for physicians in the Central West
LHIN to stay connected and encourage communication between practices.
Hosted educational sessions for a variety of patients including Prenatal day.

Provide competent and reliable in-home family support and promote healthy growth and
development of children by supporting families in their homes.

ensuring their safety at all times.
