AFFIDAVIT OF ONE AND THE SAME PERSON

We, RONNETTE O. FRANCO-ZURBITO and RHODA MARIE C. TIRANTE,
both of legal ages, Filipinos and with residents address at 31 Woodpecker
Drive, Baybreeze Executive Village, Taguig City and 24-A Umali St., Ligid Tipas,
Taguig City, after having been first duly sworn in accordance with the law
hereby depose and say:
That we personally know DATU ALLAN CONTAOI BALONO born on
February 23,1976 at Manila to the spouses Dolo Balono and Elene Contaoi and
presently residing at 44-A Eucalyptus Street, Western Bicutan, Taguig City.
That on his Certificate of Live Birth issued by the Office of the City Civil
Registrar, City of Manila under LCR No.6074 the name of DATU ALAN
CONTAOI BALONO appear on the xerox copy of which is hereto attached as
Annex A, for easy reference;
That on his Professional Regulatory Commission (PRC) and Land
Transportation Office (LTO) records the name DATU ALLAN CONTAOI BALONO
appear on the Xerox copy of which is hereto attached as Annex B, for easy
reference;
That Datu Alan C. Balono appearing on the Certificate of Live Birth and
Datu Allan C. Balono in the Professional Regulatory Commission (PRC) and
Land Transportation Office (LTO) match the entry of his birthdate refer to one
and the same person.
That we also personally know DATU ALLAN CONTAOI BALONO because
we are close family friend and officemate.
That we are executing this affidavit to the attest the truth of the foregoing
for whatever legal purpose it may serve.
