Abstract
In the report Crossing the Quality
Chasm, the Institute of Medicine asserted
that patient-centered care is one of the six
domains of quality. In this article, the
authors consider how the patient- and
relationship-centered components of
quality can be achieved in all aspects of
medical care. They suggest that faculty
development in three key areasmindful
practice, formation, and training in
communication skillsis necessary to
achieve patient- and relationshipcenteredness.

There is a thread you follow. It goes among
things that change. But it doesnt change.
People wonder about what you are pursuing.
You have to explain about the thread.
But it is hard for others to see.
While you hold it you cant get lost.
Tragedies happen; people get hurt
or die; and you suffer and get old.
Nothing you do can stop times unfolding.
You dont ever let go of the thread.
