I, DANILO H. ESCOBAR, of legal age, married, and presently residing at Brgy.

Putingkahoy, Rosario, Batangas after being duly sworn to in accordance with law, depose
and state that:

1. I am no longer employed since December 30, 2006 nor am I engaged in any kind of
business;
2. I have not been generating any income as a vendor in San Pablo City Public Market
since December 30, 2006.
3. I execute this affidavit to attest to the truth of the foregoing and for whatever legal
purpose it may serve and as a requirement by the Social Security System (SSS) San
Pablo City Branch.
