AFFIDAVIT
I, ROMMEL CALUB CANOY, of legal age, Filipino, and a resident of Davao City
after having been sworn to in accordance with law, do hereby depose and say that:
1. I am the proprietor of Herom Bodega with business address located at 213
Rodriguez St., Gem Village, Ma-a, Davao City;
2. In the course of my business operation, I am using an Isuzu Elf Dropside with
Plate Number ZSL487 that I bought from my brother-in-law last August of
2011;
3. My abovementioned Isuzu Elf Dropside is also being used by my friend Mr.
David Tatad Libre doing business under the name and guise of Shafa
Enterprise with business address at 262 Emerald St., GEM Village, Ma-a,
Davao City in his business operations;
4. I am executing this affidavit to attest to the truth of all the foregoing and to
apprise the Office of the Business Bureau of Davao City of such fact and for
whatever other legal purpose this may serve.
