AFFIDAVIT
I, DIONISIO M. SALUD , Filipino citizen, of legal age, single,
and a resident of 25 Mango St., Bel-Air Village, Makati City, after
having been duly sworn in accordance with law, hereby depose and
say:
1. That I am the Register of Deeds of Makati City since January
2006;
2. That I took a leave from office from November 1, 2012 to
February 7, 2013.
3. That for the same period, I was out of the Philippines, as I went
on a pilgrimage to the Holy Land of Jerusalem, with my wife,
Marissa S. Salud.
4. That I did not bring any cellular phone or any other mode of
communication, as my wife and I did not want to be disturbed by
any work-related concern;
5. That when I arrived in Manila on February 7, 2013, I was
received a phone call from Atty. Edwin Mendame;
6. That he informed me that he is representing Jana Monica Z.
Valenzuela in an Annulment of Title Case;
7. That I am called to testify in order to prove that a Transfer
Certificate of Title in Juanito Villanuevas name was cancelled
and a new one was issued in the name of Stephen Jacobo;
8. That Atty. Mendame has been trying to contact me since
November 7, 2012;
9. That I have explained to him my absence from the country from
November 1, 2012 to February 7, 2013;

That I am now available to testify for the said Civil Case.

Attached here is a copy of my Philippine Passport with
Passport No. XX224-510.
Further Affiant sayeth none.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 8th
