Eyes & Vision

- Eyebrows: Eyebrows are symmetrically aligned; hair is evenly


distributed and has an equal movement.
Eyelashes: Eyelashes are equally distributed, curled outward.
Eyelids Eyelids are intact. No presence of discharges. No discoloration.
Eyelids close symmetrically.
Bulbar Conjunctiva: - There is presence of observable capillaries. With
white anicteric sclera.
Palpebral Conjunctiva: - Palpebral Conjuctiva is shinny and smooth. Pink
in color.
Lacrimal gland and Lacrimal sac: - There is no tenderness or edema. No
tearing upon palpation on the inner canthus.
Cornea Sensitivity Test: the patient blink when the cornea is
touched through cotton wisps
Pupils Pupils are black in color. They are equally rounded and reactive
to light and accommodation by determining the function of 3
rd
and 4
th

cranial nerve. It constrict when looking at near objects, dilates at far
objects by determining the function of 3
rd
and 4
th
cranial nerve.
Convergence when object moved toward the patients nose at four inches
by using penlight.
- Visual fields: the patient can see objects at the periphery by letting
her cover her right eye and look directly at the students nose.
Holding the penlight in the periphery, the patient was able to view
the moving object.
- Visual Acuity: the patient was able to recognize letters by reading a
newspaper at a distance of 14 inches.
- Extra ocular muscle test: coordinated movements of both eye by
determining the function of 3
rd
, 4
th
and 6
th
cranial nerve.

Ear and Hearing
- Auricles: aligned with the outer canthus of the eye. It is firm and
there is absence of tenderness. Pinna recoils after folded.
External Ear Canal: no presence of cerumen.
Hearing Acuity Test: normal voice tone is audible by determining
the 8
th
cranial nerve.
