This has reference to our previous discussion/s and coaching sessions regarding your job performance for the
period ______________________________. After careful assessment, Management finds that you need to
demonstrate significant and sustained improvements in the following key areas of your job:
1) Attendance
2) Call handling procedure
3) Reports submission

In view of the foregoing, you are hereby sent this Notice of Performance Improvement Plan. While undergoing
the Plan, you will be required to meet performance targets within a designated review period, as set and
indicated in the Coaching Log (herein attached as Appendix A for our common reference). Such Coaching Log is
drawn in order for you to become sufficiently productive and to ensure your success in your job. We would like
to emphasize the need for you to demonstrate immediate and sustained improvement in the target
performance areas, thus expedient and speedy learning becomes imperative of you.

Management expects serious cooperation from you in terms of set behavior and job performance while
undergoing the (P.I.P). Otherwise, with reference to the employment contract you signed up with the
Company, your continued failure to meet performance standards may put at risk your continued employment
with the Company and may eventually result to your Dismissal from service. Performance improvement,
therefore, is highly sought for.

ACKNOWLEDGMENT

I hereby acknowledge that I have been given formal orientation on and was made aware about the PCCWTs
Performance Management Policy even during the early phase of my employment with the Company.

I acknowledge further that after previous discussions regarding my Failed/Poor job performance, I am
hereby subjected to undergo the Performance Improvement Plan, the guidelines and processes of which are
fully and clearly explained to me by my supervisor and other company representatives.

I acknowledge furthermore that my continued failure to meet the performance targets and completely pass
set this Performance Improvement Plan may eventually result to my Dismissal from service consistent with the
Employment Contract I signed up with the Company.
