Dear Mr./Ms. <Lname>,

Based on records available to our office, you have been absent without official leave since <dates on
AWOL> and that you have failed to officially advise our office of such absences based on standard
Company protocol.

Your failure to submit a formal request approved by your immediate supervisor as a matter of
standard operating procedure has rendered your absences unauthorized. Further, your absences
have affected the interdependent work schedule of the other members of your team, thus affecting
the Companys level of service it committed to its client.

By copy of this letter, you are advised to report immediately to our office from receipt of this notice
and explain in writing why your employment with us should not be terminated for just cause, in
violation of the Companys policy on attendance, specifically Absence Without Official Leave
(AWOL).
