The Team: 1. Inform school Wellbeing Leader of receipt of information and review process. 2. Review and discuss all schools referrals 3. Make a decision as to whether the child warrants further assessment by an Educational Psychologist, Speech Therapist or other or whether further information is required by the school. 4. Inform school Wellbeing of visit by specialist. 5. Test Student - Testing Parameters will indicate whether to pursue Submission for LNSLN Funding. 6. Write and issue a report to school indicating strengths, weaknesses and suggested ideas for catering for those needs. This is NOT a x it team or report. This team is there to give ideas and support to guide the continued learning for that child from the point of need. Share ideas in Unit meetings, peers, previous teachers to trial strategies for 4-6 weeks, reassessing for change. Keep in touch with parents. Record.

Further testing over time may be required as the gap widens between student and average achievement. The ongoing monitoring through testing and recording of minutes and plans will add emphasis to submission - if / when eligible. Each Year: Evaluate the plan that was sent in for funding to be accountable for the money we have been given and to ensure continued funding.

* Wellbeing Leader will inform when this is about to happen. * Wellbeing puts in a referral to the CEO for review of funding. * CEO allocate someone to reassess the child.
