JOINT AFFIDAVIT

We, MELANIE SALCEDO and HELEN TADIFA, of legal ages, Filipino
and residents of Cagamutan Sur, Leganes,Iloilo, Philippines , after having been duly
sworn to in accordance with the law hereby depose and say;
That, we are neighbors of ROSEVIMINDA A. ALIBOGHA, the bereaved
wife of ROD C. ALIBOGHA;
That, we personally know ROSEVIMINDA A. ALIBOGHA for almost ten
(10) years already being neighbors in the same barangay;
That, we attest that ROSEVIMINDA is the same with ROSVIMINDA and
that in ordinary undertaking she is using the name ROSEVIMINDA instead of
ROSVIMINDA as indicated in her Marriage Certificate and NSO Advisory on Marriage;
That, we further attest that the said discrepancy in the missing vowel E
in the given name was due to a typographical error;
That, we execute this joint affidavit to attest to the truth of the foregoing
declarations in relation to the provident claims / benefits of ROSEVIMINDA A.
