SCHOOL: Student: Daniel Jones Referring Teacher: Madame Lise Facilitator: Mr. King ( Administrator) Grade Level: 3 FI Date of Meeting: May 14, 2014 Time: 2:45 TEAM MEMBERS: Referring Teacher- EST-L Guidance Counselor School Intervention Worker Homeroom teacher Administrator
ISSUE(S) TO BE ADDRESSED:
The referring teacher described Daniels behaviors that needs to be addressed:
When Daniel is requested to do something, he does not want to do, he will refuse, cry or yell at the adult. He is a very capable student but will often refuse to do any writing assignments. He will blame the teacher or the class for being too noisy even when everyone is quiet and on task.
His primary issues exist outside of the classroom. During unstructured time, Daniel impulsively mistreats other children by hitting, kicking, pushing or biting. He lashes out at other students if they touch his book bag or bump into him as he does not perceive these minor incidents as accidents. Dan does not know how to let people know what he wants or needs in an appropriate manner.
He does not take responsibility for his actions. He becomes very argumentative whenever he is corrected. He will blame others by saying nobody at the school likes him. STUDENT STRENGTHS STUDENT NEEDS Strong Math Student Specific instructions for Social Skills Good Tech Skills Sense of belonging Strong Verbal Skills Take responsibility for actions Enjoys reading a variety of books and loves Lego WHAT STRATEGIES HAS THE REFERRING TEACHER ALREADY TRIED?: During circle time, Daniel sits on a chair on the peripheral of the circle where he is close enough to participate but not touching other children He keeps his belongings in an alternate area outside of the common cloakroom so others do not accidently touch his possessions. When he completes a written assignment, he is rewarded with some computer time. - Brainstorming:
. The Guidance counselor will take Daniel on a regular basis and do social stories with him that involve direct instruction for appropriate behavior.
The SIW will include Dan in the Volcano Group which is an anger management program for young children.
A lunchtime Lego club will be initiated and Dan could take on a leadership role. A reward/consequence point system (Do-Jo points) could be implemented in the classroom.
The classroom teacher will keep a weekly behavior tracking chart focusing on positive behaviors that have been noted during the week and is sent home on Friday.
As the biting/ hitting/ kicking is unacceptable and disruptive to the positive learning environment, Dan will receive an automatic one day In-School Suspension for any aggressive acts NEW STRATEGIES SELECTED: (Include who will carry out each strategy and when) STRATEGY: ASSIGNED TO: IMPLEMENTATION DATE: Social Stories Guidance Counselor May 15, 2014 Do-Jo points Classroom Teacher May 15, 2014 Weekly behavior tracking Chart Classroom Teacher/SIW May 15, 2014 In- School Suspension Administrator May 15, 2014 Lego Club SIW May 15, 2014 FOLLOW-UP MEETING WITH TEAM Date: May 28, 2014
