February 28, 2012 Re: Letter of concern for This letter is being written due to many concerns regarding your performance. Following the review of your evaluation, the improvement needed is not being demonstrated. The following areas will need to be corrected immediately. If these performance areas are not corrected we will be terminating your position. Student Involvement: Not being engaged with the students. When supervising students, has not demonstrated responsible supervision of students. You have not proven that you are able to multitask and do not initiate behavior management with students. It does not appear that students have a positive relationship with you and seldom seek your assistance. Program Guidelines: Has not demonstrated knowledge of the program guidelines. Step by step directions regarding student behavior intervention plans, level system and points sheets have had to be given repeatedly. Point sheets have been added wrong on multiple occasions and students have been placed on the incorrect level. Points have been taken away from the wrong students point sheet and you have placed a student on level 5. (No level 5 exists). Hours of work: Not staying at work for the designated time and leaving early. You have been talked to about your time sheet being completed incorrectly and inaccuracies continue. Daily Responsibilities: Needs multiple reminders and requests from the contract staff in order to complete daily tasks. You have not been successful with creating age appropriate bulletin boards. The box tops have been counted wrong after being given step by step directions on two occasions. You have not been able to accurately count 50 box tops and place them in individual bags. Building relationships with students: You have been asked many times to participate in activities with students, but you have not followed through with this request. Rather than standing in the hallway at the end of the day with the students, you choose to sit at the desk in the planning room or a classroom. It is your responsibility to participate in the check-out process with the students. Self Direction: Lacks the ability to demonstrate self-directive skills and focus on the tasks at hand. From currently needing step by step directions, to independently completing already established tasks.

We are in the business of educating students and we need staff to work to high standards in our buildings to help students reach their goals. We need to see improvement in these areas above so we can continue to meet these standards.
