    Behavior Plan

    1st warning- Teacher will verbally speak with student in regards to his/her misbehavior. Student will be told that this is the first warning.

    2nd warning removal of green card- Students will have to remove their green card from the chart.

    3rd warning removal of yellow card- Student will have to remove their yellow card from the chart. Student will be removed from activity and 5 minutes off of recess.

    Last warning, student is removed from recess for the day. A note is sent home to the parent/guardian or phone call home. Student must bring note back signed by parent, teacher keeps a copy for reference. This makes parent aware of students behavior in the classroom.

    Principal referral- If student proceeds to this final step and behavior continues, student will meet with parent, school principal and teacher.
