Consultation Report

Child: Susie Date of Birth: September 30, 2007 Date of Evaluation: September 19, 2013 School: Shades Cahaba Elementary Age: 5 years 11months and 19 days

Background and Referral Information: This child is a very patient child. She is social, enjoys her friends, and loves to smile. Her family is made up of her, her father, mother and two younger siblings who are twins. I was not privy to any pregnancy information or health issues. She did attend a Montessori preschool before switching to her current public school. Her father said that she enjoys the public school better but that the start at the Montessori provided a great start for her. He even said that she is a little ahead of her class because of it. This testing is being conducted to give the proctor practice in conducting the test. The test was at the request of the proctors professor. Test Administered: The test that was administered was the Denver Developmental Screening Revised (DDST-R). This test is composed of 125 tasks that are divided into personal-social, fine motor-adaptive, language, and gross motor areas. It is designed to test apparently well children to monitor any risks for developmental problems. Observations during testing: This test was performed in the after school care setting that Susie is accustomed to. We were sitting in a common area with few distractions. She had recently finished eating a snack and had just finished the school day. She was very willing to complete the test. She was focused on the material and tried everything that I asked of her. Test Results: Susie scored a P on all of the tasks that she was asked to perform. This outcome qualifies as normal for the Denver test. There were no opportunities for her to score an advanced score due to her age. Impression: This score of normal indicates that Susie is on track with her developmental growth. Testing conditions were good for her to perform at her highest. She was a little nervous at the beginning but after talking, she was able to perform at her best. Recommendations: Student should continue with her education placement. Signature:
