4. DSF Charitable Foundation I. Contact Information: J. Nicholas Beldecos Executive Director DSF Charitable Foundation 5840 Ellsworth Avenue, Suite 200 Pittsburgh, PA 15232 II. Proposed Narrative: The struggle to integrate those with disabilities into the community, and provide them the chance to excel despite their circumstance, is at the heart of Wasatch Adaptive Sports. Wasatch Adaptive Sports is committed to helping children and adults of all abilities. Many of our participants have a handicap of some sort and it is our goal to provide them with the opportunity to engage in recreational activities. As well as, enhance their educational knowledge of resources and options to get involved recreationally, and provide additional support that fits their ability of functioning. Disabilities range from physical, cognitive and behavioral, including providing services to post war veterans. It is our hope that we can provide education so participants are more aware of their community resources and the rights they have as an individual. Although participants might have a disability, they are still entitled to exercise their rights on an equal basic with others. Often those with disabilities are treated as unequal and it is our passion to help provide a sense of belonging and ownership within their own life and community. The goal of Wasatch is to help provide the participants with skills that they can use to help increase the quality of their life and enhance their community engagement, potentially helping others and benefiting the nation on a smaller scale. Wasatch Adaptive Sports works on trying to enhance three areas: health by providing a recreational experience, human services by working with those children and adults with disabilities as well as veterans, and education by providing new resources through mentorship, teaching new life skills and news ways of participating in activities by providing programs in which all this can be achieved. This is consistent with the DSF Foundations three focuses, health, human services and education and their commitment to focus on developing life-skills, mentoring, and youth-development programing. We are requesting a grant to help fund our downhill Skiing and Snowboarding program for children and adults with special needs. Adaptive equipment will be provided based on individual needs. We are trying to fund the equipment to help meet the needs of each individual. Equipment that we want to have available for our participants includes sit-down skiing devices, outriggers, ski-walkers, hula hoops, and regular ski and snowboard equipment. Due to the specialized nature of the equipment it can be costly to supply. We are estimating the cost of equipment to be $15,000. If you are interested in seeing the financial budget that lays out the cost of equipment and how the money would be specifically spent, please contact us with a request. One of the main goals of our organization is to help fill a need in Utah for affordable recreational and educational programs for those with varying disabilities. The programs build strength, stamina and self-esteem which enhancing the quality of life for all participants. The hope is with this increased strength, stamina, and self-esteem the participants will be able to give back to their community after receiving from this positive program. We also want the participants to gain skills so they can compete in the Paralympics and compete not for fame and recognition but to be an advocate for adaptive sports and support of helping those with impairments, bridging the gap between the two communities. Our program is evaluated by participant satisfaction and feedback. We work with our individuals on a one-on-one need bases so at the end of their time with Wasatch or the conclusion of a program, the participant or adult supervisor will fill out an evaluation to help us enhance our program.

The program will demonstrate success if after each session or the completion of the program the individual has higher results and outcomes than their initial assessment. The initial assessment will asses the strengths and weaknesses of the client, create goals and objectives for their time in the program, work to help educate the client on an individual bases of opportunities they have available and see what they want to accomplish and get out of the program. At the end, if the individual has more functional ability, is stronger, increased their endurance, is more self-confident and assure of their skiing ability the program will demonstrate success. Success will also be measured based off if the participants achieved their desired outcome to the initially set goals and objectives and how they contribute back to the program through either, continued participation, volunteering to help others in the program, or competing in the Paralympic games. The downhill skiing program is based off the individual and will be seen as successful if the concluding assessment shows the increase in the individuals health, community interaction, education and access to resources. The goals of the granting organization is to provide human and community services, Wasatch provides human and community services, while providing programs for a large range of participants from children with disability to veterans, which matches the goals of the granting organization. The goal of DSF Charitable foundation is to create sustainable models of excellence that have potential to be replicated elsewhere and be of more general benefit to the nation. With the purchase of the equipment and minimal payments all income from participants will go to enhancing the program or to provide scholarships. The funding will help increase the popularity of the program and the number of participants, helping to get more people invested, both those who are benefiting from the program, and those invested in helping provide scholarships and financial support so this program can maintain the reputation of not turning people down due to financial issues. The need from you is to provide a grant that provides equipment so the participants have the opportunity to learn, train and compete in these organizations. The grant money will be invested in equipment that will last for seven years or more. Giving us several years to create a credible reputation and build up our program before needing new equipment. This kind of equipment is build well and will be treated with special care to insure the maximum benefits and usage. Because our program has a large focus on helping Veterans, Wasatch partnered with the VA Adaptive sports organization. They are helping establish us as a national organization, registering us at the regional and national level. Having Paralympic Sports would help our programs get national recognition and provide services that go beyond the Utah area, providing potential for success in other parts of the country and in other areas. Our organization takes pride in our equipment and if treated properly, it will last for a number of years. The equipment is a key component in training of the participants and helping work on meeting the goals of the organization, therefore helping lead to a successful community program. Wasatch Adaptive Sports believes strongly in its mission to provide services to those who are often left out and to help build a program that provided individual growth and a chance to excel in a new way. Many time people are placed into situations that are beyond their control, helping them realize their community resources and provide new opportunity is what we believe in. We know with the help from the DSF Foundation and a grant for the adaptive equipment, our program can help enhance many lives and give back to our community.
