Employment Project Background: The Employment Project will require you to complete 10 hours of community service/ wellness activity

each quarter. You may fulfill your project requirements through: o Health-related volunteering (via hospital, clinic, doctors office, etc.) o Community service (via health nonprofits, HOSA, Club Med, etc.) o Work-based learning (ongoing internship at a health-related facility) o Wellness/Advocate Healthy Living Activities (Fit Club, JDRF Walkathon) Work-based learning at one or two facilities throughout the entire school year is strongly encouraged as you will be given greater responsibilities the longer you stay at one place. You will also develop stronger bonds with employers and a deeper knowledge of workbased practices and expectations. Requirements: 5 hours (minimum) of health-related volunteering, community service, or workbased learning

o 5-slide Work-based Evidence PowerPoint: **(optional )

One log for every two hours of volunteering / service / work Signatures of supervising adults on each work log One slide for every two hours of volunteering / service / work Each slide should have pictures of you in action Brief text on each slide explaining what youre doing

o One-page Work-Based Critical Thinking Reflection: **(required)

The method of submission is portfolio. All documents should be submitted electronically (preferred). The order is as follows: 1. Cover Page (Student Name and Period) 2. Work Logs 3. Work-based Evidence PowerPoint 4. Work-based Critical Thinking Reflection

Employment Project Rubric 5 (A) The Employment Project Contains All of the Following: 10 hours or more of health-related volunteering, community service, or work-based learning Work logs are complete, honest, and detailed Work-based Critical Thinking Reflection is highly detailed, demonstrating deep introspection and self-analysis 4 (B) The Employment Project has Only One or Two of the Following Flaws: Hours requirement was met, but a few work logs were not highly detailed Work-based Critical Thinking Reflection is moderately detailed 3 (C) The Employment Project has Only Three or Four of the Following Flaws: Hours requirement was met, but a few work logs were not highly detailed Work-based Critical Thinking Reflection is moderately detailed 2 (D) The Employment Project has Only Five or Six of the Following Flaws: Hours requirement was met, but a few work logs were not highly detailed Work-based Critical Thinking Reflection is moderately detailed OR ONE SEVERE ERROR SUCH AS: Hours requirement NOT met No Critical Thinking Reflection
