AFFIDAVIT OF DENIAL
I, JEFFREY S. MARTINEZ, of legal age, Filipino citizen and with address at 513 Villaramirez, Tabon I, Kawit, Cavite under oath, states, that: On May 3, 2012, while I was securing a certification of no pending case, I was informed that a case for Robbery with Violence Against or Intimidation of Persons was filed against a certain JEFFREY MARTINEZ. Upon verification, I came to know that the case is pending before Branch 21 of the Regional Trial Court with Criminal Case No. 4982-08 and the address of the said person is Phase 4, Dasma I, Golden City, Subd., Salawag, Dasmarinas City, Cavite. I am NOT the person subject of the above criminal case. To prove the same, attached is my Authenticated Birth Certificate (Annex A) and my picture (Annex B). This is to prove that the picture attached and made part of the records of the above criminal case is not me. Also, I have never been a resident of Salawag, Dasmarinas City, Cavite as I am a bonafide resident of Tabon I, Kawit, Cavite. I was never involved in any robbery incident during my entire stay in Cavite. I attest the truthfulness of the foregoing statements in order to clear my name.
