COMPLAINT AFFIDAVIT

I MAILENE A. CAPARROSO, of legal age, with postal address at WYNESONS FINANCE Inc., Rm. 310 ANITA Bldg. South Ave. cor. Quezon Ave. South Triangle, Quezon City 1300, after having sworn in accordance with law do hereby depose and state: I am the Legal and Collection Assistant of the private complainant corporation, WYNESONS FINANCE INC., with address at Rm. 310, 3rd Flr. ANITA Bldg. South Ave. Quezon Ave. South Triangle, Quezon City 1300 which is duly authorized by the Banko Sentral ng Pilipinas as Lending Investor. That on, February 28, 2010, MS. FLORDELIZA M. MACASPAC with residence address at, 749 CUPANG MUNTINLUPA CITY obtained a loan from WYNESONS FINANCE CORPORATION INC., with the amount of PHILIPPINE PESO: EIGHTY THOUSAND PESOS ONLY (Php, 80,000.00). That in payment of said FINANCIAL OBLIGATION. MS. FLORDELIZA M. MACASPAC, issued several posted check / on same date which when deposited to our account was retuned with annotation of ACCOUNT CLOSED which has been issued at WYNESONS FINANCE Inc., with office address at Rm 310. 3rd Flr. ANITA Bldg. South Ave. cor. Quezon Ave. South Triangle, Quezon City 1300 Among the check/s issued by the respondent are the following: BANK CHECK METROBANK 2412748606 AMOUNT P35,000.00 DATE 05/04/11 ANNEX A1& A2

That said company sent a letter of demands to MS. FLOREDIZA M. MACASPAC, requesting him/ her either to replace the above check/s with cash or current check/s including a Notice of Dishonor of check and is hereto attached and was marked as Annex B. That despite repeated demands, through letters and telephone calls, MS. FLOREDIZA M. MACASPAC, FAILED AND REFUSES TO SETTLE his/her accountabilities with the company. This affidavit is executed to institute a CRIMINAL CHARGE against MS. FLOREDIZA M. MACASPAC for violation of Batas Pambansa Bilang 22. MAILENE A. CAPARROSO AFFIANT
