Now comes Gary E. Horn, being first duly sworn according to law, says that he is the attorney for the Estate of Leona Bayn Farrell, Deceased. Affiant further says that Leona Bayn Farrell died a resident of Toledo, Lucas County, Ohio, on the 20 day of April, 2012, and that no administration proceedings are being had in the Estate of the said decedent, in the Probate Court of Lucas County, Ohio.

Affiant further says that Leona Bayn Farrell and Oren Johnston Farrell acquired title to the hereinafter described real property located at 913 Garth Street, Toledo, Ohio 43613, by a deed dated April 13, 1978, and recorded in Toledo, in the Lucas County Deed Records, as Joint and Survivorship Deed, pursuant to R.C. 5302.17. The legal description thereof is:

Lot Number 486 in the Lincoln Woods subdivision, Lucas County, Ohio.

Prior instrument Reference: Deed Record No. 78-1031B02, Lucas County Deed Records. By reason of the death of Leona Bayn Farrell, the survivor, Oren Johnston Farrell, is now vested with sole and complete title to the properties herein described.
