DELANTAR, both of legal age, Filipinos, and presently residing at Brgy. 06, Balangiga, Eastern Samar, Philippines, and after having been sworn to in accordance with law, hereby depose and state that: 1. Since childhood, we have known ROLANDO TERCEO DEGORIO of Balangiga, Eastern Samar, Philippines; 2. He was born to RESTITUTO A. DEGORIO and AMALIA TERCEO, on September 02, 1960 in Balangiga, Eastern Samar; 3. Based on the data appearing in his birth certificate issued by the Office of the Civil Registry of Balangiga, Eastern Samar, his name is spelled as ROLAND TERCENIO DEGORIO, with date of birth September 02, 1960; 4. In his personal records, transactions, and communications, he had been using the name ROLANDO TERCEO DEGORIO; 5. The name ROLAND TERCENIO DEGORIO born in Balangiga, E. Samar, and ROLANDO TERCEO DEGORIO born in Balangiga, E. Samar, refer to one and the same person; 6. We are not related by consanguinity or affinity to ROLANDO TERCEO DEGORIO; 7. We are executing this affidavit to attest to the truth and veracity of the foregoing statements in compliance with the requirements of correcting clerical errors in his Birth Certificate, i.e. from ROLAND TERCENIO DEGORIO to ROLANDO TERCEO DEGORIO, under R.A. 9048, and for whatever legal purpose this affidavit may serve.
