Ogoc, a resident of Liangan East, Bacolod, Lanao del Norte, after having been duly sworn to an oath in accordance with law, hereby depose and say: 1. That I was born JEANIFER BOLABOG JAUCULAN on July 19, 1971 at Calican, Pian, Zamboanga del Norte to the spouses Fortunato Jauculan and Maria Bolabog; 2. That I was married to Oseas T. Ogoc on April 6, 1990 at Batuan, Bohol and my name reflected in our Marriage Contract is JENIFER JAUCULAN; 3. That we were blessed with five children and the notable discrepancies of my name as reflected in their respective Certificates of Live Birth are as follows: Jemalyn born on January 2, 1991 my name is JENIFER JAUCULAN, Marie Joy on September 5, 1993 JENIFER BOLABOG JAUCULAN, John Lloyd on October 22, 1999 JENNEFER BULABOG JAUCULAN, John Carlo on October 22, 1999 JENNEFER BULABOG JAUCULAN, and Hanz Michael March 7, 2011 JENIFER JAUCULAN; 4. I was not known by any other name in our locality, nor has there been any person in our locality similar to my name; 5. That the names JEANIFER BOLABOG JAUCULAN as reflected in my Certificate of Live Birth, JENIFER BOLABOG JAUCULAN in my Marriage Contract and three of my children, and JENNEFER BULABOG JAUCULAN in my twin children refer to one and the same person and that the one that I am using is JENIFER BOLABOG JAUCULAN as reflected in my Marriage Contract; 6. That I am executing this affidavit to manifest the facts abovestated and to support the retirement claims of my husband and for other legal intents and purposes devoid of malice whatsoever.
