Annual Agreement School Counselor: Sarah Cook Year 2012-2013 Job Description: The counselor at Old Saint Marys

position will include, but will not be limited to: Develop a comprehensive school counseling program that addresses all parts of the ASCA national model. The compressive model will utilize individual and group counseling, crisis intervention, conflict resolution, outside referrals, and academic support. Development and implementation of a health curriculum for appropriate grade levels. Lead Response to Intervention by identifying students who require additional support. Develop a resource file of evaluation services that can be made available to families. Arrange conferences with parents, teachers and administration to address concerns, progress, or changes in intervention. This process will require the counselor to maintain records for meetings with pertinent documents and information, which will allow them to follow up with all stakeholders. Develop programs and actives for students who require additional challenges to ensure their needs are met. Develop and implement a high school transition program for all middle school students. This will involve becoming familiar with all of the Catholic high schools available to our students, arranging high school visits, shadowing days, and assisting students in the application process. Assist with academic planning and curricular work by assessing test data, interpret assessment results, and develop materials for parents to easily understand test results. Be a member of the Leadership Team and develop a process for evaluation of the program and the counselor.

School Counseling Program Mission Statement The school counselor at OSM aims to help students become successful lifelong learners by working collaboratively with staff, parents, community, and the students to ensure that the academic, career, and

personal/social needs of all students are met. The OSM school counselor aims to provide a comprehensive school counseling program that centers around an engaging and rigorous curriculum that encourages high levels of achievement and that also helps to build a solid foundation in the Catholic faith. School Counseling Program Goals The school counseling program will focus on the following achievement, attendance, behavior and/or school safety goals this year. Details of activities promoting these goals are found in the curriculum, small group and closing-the-gap action plans. Program Goal Statements 1 By the end of a four-week small group intervention on body image and self-esteem, 7th grade girls will increase their self-esteem and confidence by two integers as evidenced from the post-test. 2 6th and 7th grade students will decrease their incidences of bullying by 50%, as evidenced from longitudinal data collection. 3 5th grade students will utilize stress management strategies gathered from closing the gap interventions to decrease the percentage of students who report feeling stressed before a test by 10%, evidenced by a post-test.
