YB World Taekwondo Academy Program Agreement Policy

PROMOTION EXAMINATION In order to advance in rank, the BUYER/MEMBER must take promotion examinations at least once a year. In order to cover the costs of registration, promotion, certificates and new belt, a testing fee is charged at each examination. All Promotion examination fees and protective equipment are extra and not part of this contract. All TEST applications are due one (1) day prior to test date. Official Belt recognition and certification will be issued upon the successful completion of examination INSTRUCTION I understand that under the terms of this Agreement, YB's WORLD MARTIAL ARTS ACADEMY obligates itself to furnish me with instruction and suitable facilities for teaching lessons. All Classes are supervised by qualified personnel who are trained in the procedures and of the Martial Arts. I also understand that strict observation of the rules and regulations relating to training include the use of protective equipment required by the Institute. . I hereby represent that I am in good health and physically fit to take the prescribed course of instruction and that I have had an opportunity to observe and participate in the above-described art of self-defense prior to the signing of this agreement. I have listed on the back of this form any medical conditions which may be pertinent to my enrollment. ATTENDANCE I understand that it is my responsibility to attend class and follow instructions. It is further understood that I am obligated to make payments whether or not I attend classes. I further understand that failure to complete lessons does not relieve me of my obligation to pay the tuition in full. INJURIES The required and suggested equipment is designed to decrease the possibility of accident or injury; however, YB's WORLD MARTIAL ARTS ACADEMY does not warrant the protective equipment. I understand that during the course of instruction, employees of YB'S WORLD MARTIAL ARTS ACADEMY and I or other students or authorized persons will be engaged in a course of conduct requiring physical contact and training including, but not limited to kicking, punching, stretching and calisthenics, running, and jumping. I understand and agree that YB's WORLD MARTIAL ARTS ACADEMY will not be liable nor held responsible for any injuries, damages, losses, claims, or causes of action not caused by or resulting from the negligence of the owners, operators, or persons in charge of such establishment, or their agents, servants or employees. I acknowledge reading and receiving a copy of this agreement and fully understand its terms. Nothing in this provision should be construed as displacing applicable laws or rights regarding liability. CANCELLATIONS As required by General Obligations Law, I have certain rights to cancel this contract. These are set forth indicated below. NOTE: A $20.00 late charge will be assessed for any payments ten days past due. I understand my rights as stated above. *CONSUMER'S RIGHT TO CANCELLATION: You may cancel this contract without any penalty or further obligation within three (3) days from the date of this agreement. All course fees paid or due are non-refundable six (6) months after the date of this agreement. Notice of cancellation shall be submitted by the official school form to YB'S WORLD TAEKWONDO ACADEMY by registered or certified mail and the payee is charged with (1) month cancellation fee. If this agreement is funded by payment of credit card, you (payee) are subject to a (2) months cancellation fee. MISSED CLASSES: Barring medical reasons, missed classes are the full responsibility of the student to make up before the end date of the agreement. An official school form must be submitted prior to any vacations or leaves that last more than one (1) month in order to have credited classes to the agreement. AGREEMENT Being a member of the school, I will faithfully comply with all the rules and regulations of the school and the traditions of the martial arts. Failure to take the lessons in the allocated time, without written request or approval, invalidates the lessons beyond the expiration date. I further understand that failure to complete the lessons beyond the expiration date does not relieve me of my obligation to pay the tuition in full. Lessons are not conducted on national holidays. I also understand that pictures and videos will be taken at selected events and classes by YB staff and will be used for official purposes. If I do not wish my child to be involved in any kind of media used by YB World, I will contact the front desk and let them know. Permission must be given by YB staff in order to take pictures at events and/or classes of students other than my child. TRANSFERRING MEMBERSHIPS: Memberships may be transferred between family members or friends. An official form must be submitted in order for the agreement to be transferred. Only the full length (or remaining length) of the agreement may be allowed to be transferred over to the prospective student. REFUNDS Money refunds are only available to those who are ending the agreement due to: Medical Reason: A signed Doctors note stating the student is no longer able to safely practice martial arts is required in addition to the official school form. Change of Address: If the student moves to a location that does not have an YB World Taekwondo Academy within a (20) mile radius, then the student is qualified for a money refund. A document of proof and the official form is required in order to be considered for a money refund. All other refunds must be in the form of credited classes to the student.
