Dear Sir/ Madam,

This letter of indemnity is being submitted as per your instructions: I Mrs., Travelling by Flight AI 111 Del-Lon,on xx September xxxx, am suffering from and would require in flight oxygen support at the rate of 4 L per minute through the duration of the flight, which has been approved by your Doctor graciously, I understand that Air India will be only providing me the oxygen supply, comfortable seating with leg space near toilets and wheelchair assistance and no medical expertise will be available in case of an eventuality during flight. With such understanding, I indemnify Air India against any such occurrence arising out of my condition. This shall apply to my return flight A I112, Lon-Del on October xxxx as well. My Husband, Mr .. is aware and agrees to the submission as above.
