AFFIDAVIT OF WITNESS

I, Mijandro Manukol, legal age, single, resident of Brgy. Sto, Domingo, Quezon City, after having sworn in accordance with law, do hereby depose and state THAT:

I am employed as a Security Guard of OMC Carrier at No. 99

Kalayaan, Ave., Quezon City; 2. I was on duty guarding the premises of OMC Carrier from 9:00

p.m. up to 7:00 a.m. in the 29th of June 2012; 3. While I am seated at my post, Roderick Awa approached me and

asked for the time and without a warning poked a gun on me and knocked me unconscious by using the bottom of the gun; 4. I regained consciousness and found out that the Isuzu Tanker with

a plate number TMG-390 parked inside the premises of OMC Carrier was already missing; 5. I immediately informed Mr. Chito Miranda of the incident and we

both went to the Police station.

I executed this affidavit to attest the truthfulness of the foregoing facts.
