1.  Your patient is a 27 year old female who presents with a several month history of progressively worsening fatigue.  Lab evaluation reveals a hemoglobin of 9.6, hematocrit of 28.1, and a guiac that is negative.  Her further workup reveals a macrocytic anemia.  She ensures us that she eats a health diet along with a vitamin supplement, despite having a low Vitamin B12 on lab evaluation.  What hormone may she be lacking?

A.  Gastrin

B.  Intrinsic Factor

C.  Cyclooxygenase

1. The correct answer is B intrinsic factor.  This type of anemia, pernicious anemia, is caused by the lack of intrinsic factor being secreted by the parietal cells of the stomach.  If a patient is lacking in intrinsic factor the Vitamin B12 should ideally be administered parenterally.   Another common nutritional deficiency that manifests itself as a macrocytic anemia is folic acid.  This is common in patients that have had gastric bypass and who are pregnant.  A common microcytic hypochromic anemia is iron
