LOST IN THE WOODS

For years, I have preached the gospel of COAF, an aviation acronym for “combination of adverse factors.” Simply, COAF defines the series of missteps that precedes an accident, such as inclement weather, inadequate training, poor flight planning, or an overloaded airframe.
